Using data from the National Practitioner Data Bank (an electronic collection of malpractice settlement payments made to practitioners in the US since 1986) approximately 80,000 to 160,000 patients suffer permanent disabilities from misdiagnosis each year.

The most common diagnosis safety rule violations were missed diagnoses, compared to those that were late or incorrect. Overall diagnostic safety rule violations, including incorrect, missed or delayed were most likely to result in death than other types of malpractice, such as surgical mistakes or medication overdoses.
